EPO Membership Renewal Application

Please fill out the form below in order to complete your Eldercare Professionals of Ohio renewal application.
After submitting, you will be forwarded an invoice to pay for membership. Please allow approximately 48 hours to receive. Membership is not complete until invoice is paid.

*IMPORTANT INFORMATION: We are in the process of transitioning our membership cycle. At this time there are two choices for payment. (1) Pay only the prorated amount for the rest of 2020. (2) Pay the prorated amount for 2021 plus the cost of the entire year of 2021 and receive a 5% discount.

Membership Type

$100 Membership Level I - Attendance for one individual from an organization at one monthly networking event on either East or West side, Link on EPO website, Free placement on calendar of events & career board, One free visitor pass.

$175 Membership Level II - Attendance for two or more individuals from an organization at one or more monthly networking event on either East or West side, Link on EPO website, Free placement on calendar of events & career board, Two free visitor passes.
